Gallstones: When Do You Need Surgery and When Is It Safe to Wait?

Your Ultrasound Says “Gallstones.” Do You Automatically Need Surgery?

Not always.

This is one of the most common questions people ask after an ultrasound report suddenly mentions gallstones.

If gallstones are discovered accidentally and are not causing symptoms, treatment may not be required. These are often called silent gallstones. However, once gallstones begin causing repeated attacks or complications, gallbladder surgery may become the recommended treatment. NIDDK

So the important question is not simply:

“Do I have stones?”

It is:

“Are these stones causing a problem?”

What Are Gallstones?

The gallbladder is a small organ located beneath the liver. Its main role is to store bile, which helps with the digestion of fats.

Gallstones are hardened deposits that form inside the gallbladder.

They may be tiny or considerably larger, and some people have one stone while others have many.

Gallstones can exist silently for years.

The problem often begins when a stone blocks the normal flow of bile.

What Does a Gallbladder Attack Feel Like?

One of the classic symptoms is pain in the upper-right side of the abdomen.

It may also be felt in the upper-middle abdomen.

Gallbladder attacks can last for hours and often occur after heavier meals. Some patients may also experience nausea or vomiting. NIDDK

Patients often describe the experience as:

"I ate dinner normally, and suddenly I had severe pain below my ribs."

Because abdominal pain can arise from many different organs, gallstones should not be diagnosed from symptoms alone.

When Can Gallstones Be Left Alone?

Imagine you undergo an ultrasound for an unrelated reason.

The report says:

“Multiple gallstones.”

But you have never experienced a gallbladder attack.

You have no fever.

No jaundice.

No recurring upper abdominal pain.

In many people with truly asymptomatic or silent gallstones, immediate treatment may not be necessary. NIDDK

Your doctor will assess the complete clinical picture and advise whether observation is appropriate.

This is why an ultrasound finding alone should not trigger panic.

When Is Gallstone Surgery Usually Recommended?

Surgery becomes more relevant when gallstones are causing symptoms or complications.

The standard surgical treatment is cholecystectomy, which means removal of the gallbladder.

Common reasons to consider surgery include recurrent gallbladder attacks or complications related to stone blockage. NIDDK

Waiting after repeated attacks may simply mean repeatedly facing the same unpredictable pain.

What Is Laparoscopic Gallbladder Surgery?

Today, gallbladder removal is commonly performed using laparoscopic or keyhole surgery.

Instead of one large incision, surgeons operate using smaller incisions and specialised instruments.

For appropriate patients, laparoscopic cholecystectomy often allows a shorter hospital stay and faster recovery than traditional open surgery. NIDDK

However, every operation should still be approached as a medical procedure with potential risks and benefits that need to be discussed with a surgeon.

Can You Live Normally Without a Gallbladder?

Yes.

The gallbladder stores bile but is not essential for survival.

After it is removed, bile continues to travel from the liver into the intestine.

Most people can return to a normal lifestyle following recovery. A small number may initially notice softer or more frequent stools, which are often temporary. NIDDK

This means you do not need a gallbladder to digest every meal.

Your digestive system adapts.

Can Gallstones Be Dissolved Without Surgery?

This question is particularly popular because many people understandably want to avoid an operation.

However, non-surgical gallstone treatments are generally reserved for selected circumstances.

Certain medicines may dissolve some cholesterol stones, but treatment can take months or years, and stones can return. ERCP may be used when a stone is lodged in the common bile duct, but that is different from simply removing stones from the gallbladder while leaving it behind. NIDDK

Be cautious about online products promising to "flush out" or dissolve all gallstones naturally.

If gallstones are causing repeated problems, seek proper medical evaluation.

What Happens if a Gallstone Blocks a Duct?

This is where gallstones can become more serious.

Persistent obstruction may contribute to complications involving the gallbladder, bile ducts or pancreas.

Seek prompt medical attention if abdominal pain is accompanied by:

●     Fever or chills

●     Persistent vomiting

●     Yellowing of the eyes or skin

●     Dark urine

●     Pale stools

●     Severe or prolonged abdominal pain

These may signal infection, inflammation or obstruction requiring urgent evaluation. NIDDK

Is Diet Enough to Prevent Another Gallbladder Attack?

Diet can influence symptoms in some people, particularly if heavy or fatty meals consistently trigger discomfort.

But dietary modification cannot reliably make existing gallstones disappear.

If stones repeatedly obstruct the gallbladder and cause attacks, simply avoiding certain foods may not solve the underlying issue.

Think of diet as part of overall health management—not a guaranteed replacement for appropriate treatment.

How Do Doctors Decide Whether You Need Surgery?

The decision usually considers:

Your symptoms, number and pattern of attacks, ultrasound findings, evidence of inflammation or complications, other medical conditions and your overall surgical fitness.

This explains why two people with similar ultrasound reports may receive different recommendations.

One may safely be monitored.

The other may require surgery.

Treat the patient, not just the scan.

What Should You Do After Gallstones Are Found?

Don't panic.

But don't ignore repeated symptoms either.

If you have painless gallstones found incidentally, discuss whether monitoring is sufficient.

If you experience recurring attacks, speak to a general or laparoscopic surgeon about whether laparoscopic gallbladder surgery is appropriate.

At Hycare Hospitals in Chennai, gallstone evaluation can help patients understand whether they require observation, further testing or surgical management.

Do all gallstones require surgery?

No. Asymptomatic gallstones often do not require immediate treatment. NIDDK

Gallbladder removal, or cholecystectomy, is the usual definitive surgical treatment. NIDDK

The gallbladder itself is removed, so gallstones cannot recur within it, although uncommon bile-duct stones can still occur

Yes. The gallbladder is not an essential organ. NIDDK

Severe persistent pain, fever, vomiting or jaundice requires prompt medical evaluation
